Case Managers/Service Coordinators coordinate services across all program and agency lines. Following the Service Plan for adults and Individual Family Service Plans for children and families, the Case Manager/Service Coordinator coordinates and monitors the services to ensure that the person’s needs are met. Case Management also makes a developmental disability determination, assists in transitions to and from school programs and manages the list of persons waiting for services.
Case Managers/Resource Coordinators also are responsible for enrolling individuals into services. For many this involves enrollment into Medicaid Waivers and yearly assessments to determine continued eligibility for these programs.
